**Must reside in the state to qualify for position** Position Summary: The primary duties of the Crisis Mobile Outreach worker (CMO) are to be on standby during weekend hours. In the event a crisis occurs and the IDDD Crisis Hotline is called, the CMO worker would be contacted by a CARE Hawaii Crisis Shelter staff member. The staff member would update the CMO with all important details regarding the call. The CMO worker would have 45 minutes to arrive at the location of the crisis. Upon arrival, the CMO worker would provide face-to-face support, establish rapport, and be a support network for both the care provider and the participant in crisis. Once the participant is stabilized, the outreach would be completed. The CMO Worker is responsible for completing a crisis assessment form and submitting the form to the IDDD Crisis Resident Manager. Other duties include weekend shift coverage at the IDDD Crisis shelter on weekends as needed. Position Qualifications: • Minimum Education: Bachelor’s Degree, at minimum, in social services, psychology, human development, family sciences, or other related degree • Minimum Field-of-Expertise: One-and-a-half (1.5) years’ experience working with individuals with ID/DD and/or behavioral crisis. • Required Certification/Registration: Basic Life Support: (current & renewed prior to expiration o CPR and First Aid o MANDT and/or CPI o PBS and Person-Centered Planning • Preferred Experience: Work experience with adults or children with developmental and/or intellectual disabilities Duties and Responsibilities: • Open availability on weekends, and/or 24 hours a day, and/or 7 days a week, and/or including holidays. • Ability to cover shifts as needed at the crisis shelter. • Must have reliable transportation, be able to transport participants to crisis shelter if needed and must be able to arrive at the location of crisis within 45 minutes. • Must have a compassionate and supportive attitude that aligns with CARE Hawaii’s mission and vision. • Ability to establish rapport with care providers and Individuals in a state of crisis. • Must be comfortable modeling redirection methods and giving care providers useful de-escalation strategies that can be useful for future crisis situations. • Provide next day follow up via phone call to ensure care providers feel supported. • Ability to accurately document and log CMO events after completion • Acts as liaison and consults with ID/DD Crisis Shelter Resident Manager, Program Director, DOH Case Management. 2 CARE Hawaii, Inc. DD03 DD CNS Emerg Shelter Crisis Direct Care Staff Job Description • Responsibly reports to work on time/consistent attendance and follows program protocol when calling in sick and or for emergencies. • Must be able to assess and consult with family, caregiver, or program staff for advice; assess for need for medical or psychiatric consults including follow-up with CM/Circle of Support. • Must be able to coordinate with other agencies, such as police or emergency personnel including follow-up with CM/Circle of Support. • Must be able to conduct an overall assessment of the participant, situation, and environment. • Must be able to create a plan to implement with participant and circle of support and provide person-centered plan moving forward which includes but is not limited to, safety plan, appropriate information, referral, contact information, follow-up, etc. • Must be able to assess any suspected case of abuse (psychological, sexual, financial, neglect) and report to Director and Protective Services (Adult or Child Welfare Services) immediately upon discovery. • Must be able to attend all supervisions, trainings, meetings, etc. Job Competencies Required Skills: Mature, reliable, and responsible. Working Environment: Indoor/outdoor, open windows or air-conditioned environment; require some travel, over-time, weekend, and holiday work, working with other Federal and/or State accounting bodies, noise level in the environment may be moderate to noisy. Exposure to infectious diseases is a relatively common risk. Special Demands: Must be able to work as the ID/DD emergency shelter on a scheduled shift. Ability to respond effectively to the varied needs and demands of multidisciplinary team members. Have access to helpful, adequate, competent, and continuous supports and services. Knowledge of how to recognize and act upon a lifeor-death situation.
