Your Role and Responsibilities:
As an Intake Coordinator, you will play a vital role in our organization by managing the intake process for new clients. You will be responsible for assessing eligibility, coordinating with referral sources, and ensuring a smooth transition for clients into our care. Your excellent communication skills and attention to detail will be essential in building positive relationships with clients and referral entities.
Key Responsibilities:
- Eligibility Assessment: Review and evaluate client eligibility for services based on established criteria.
- Communication: Serve as the primary point of contact for referral sources, providing timely and accurate information.
- Documentation: Maintain thorough and accurate records of client intake processes and communications.
- Collaboration: Work closely with our care team to ensure a seamless transition for clients into our services.
- Follow-Up: Conduct follow-up communications to gather additional information and address any questions or concerns.
- Admission Coordination: Manage the coordination of admission, assessment, and referral of incoming patients, including coordinating provider assessments and initiating the admission process. Provide technical assistance to the discharge planning process.
Qualifications:
Education and/or Experience:
- Working knowledge of office practices and procedures.
- Strong computer/data entry and software skills.
- Some medical terminology or healthcare industry experience preferred.
Language Skills:
- Must possess good communication skills, both written and verbal in English.
- Ability to communicate in other foreign languages is desirable.
Mathematical Skills:
Reasoning Ability:
- Able to work under pressure and handle multi-task functions.
- Must be able to deal effectively with people.
- Must be self-motivated and resourceful.
Physical Demands:
- Physical demands for this position include the ability to be seated for extended periods of time and use computers/office equipment.
