Nurse Practitioner Position Overview:
We are seeking a dedicated Nurse Practitioner to join our healthcare team. As a Nurse Practitioner, you will play a crucial role in providing evidence-based, patient centered primary care to patients with intellectual and/or development disabilities. This position offers the opportunity to work in a dynamic medical environment.
Time Commitment: Seeking part-time or full-time employment. Benefits are offered based on minimum commitment of work hours per week.
WIHD provides the broadest array of health services and medical specialties in Westchester and beyond to serve the needs of individuals with developmental disabilities. In addition to primary care, we offer many behavioral health resources including psychiatry, neurology, and behavioral psychology. WIHD offers appointments with medical specialties from cardiology to podiatry, as well as comprehensive dental care. We evaluate and meet our patients’ needs for shoes, braces, and wheelchairs. We don’t just treat diseases — we promote health and wellness in our patients.
Primary Responsibilities:
- Provides patient centered primary medical care by taking full patient histories and performing examinations in order to develop and implement a plan based on diagnostic and demographically specific evidence based guidelines. This process includes, but is not limited to: ordering and reviewing diagnostic labs and imaging; ordering and reviewing consultations; administering procedures including shots, as needed, communicating with patients and caregivers regarding their individualized care management plan; collaborating with internal and external care providers; providing appropriate healthcare instruction and education; and documenting all healthcare communication and action in the electronic medical record.
- Collaborates with the care manager in stratifying primary care patients according to medical, behavioral, and social determinants of health using population management tools in the electronic health record. Also stratifies patients according to the number and type of internal/external referrals and their overall healthcare utilization to identify high risk primary care patients in need of care management plans.
- Participates in clinical quality initiatives and meetings.
Qualifications:
Education: Graduation from an approved Nursing Program with a Master’s degree in a relevant area of specialization (i.e. Family Practice); Possession of a license to practice medicine in New York State; Board Certified Family Nurse Practitioner;
Knowledge of: Electronic medical records; Awareness of current local, state, and federal laws governing the delivery of care.
Kind and Length of Experience: Minimum of three years’ experience as a Nurse Practitioner.
Experience caring for individuals with developmental disabilities is preferred.
