Registered Nurse Admissions – Treatment Center - We are seeking a Registered Nurse who is passionate about caring for patients and helping them live healthy, fulfilling lives. We are seeking a driven individual with the ability to contribute to our clinical and technical teams.
Responsibilities:
- Performs or delegates the initial and ongoing evaluation of patient needs within their scope of practice.
- Routinely completes Start of Care (SOC), Resumption of Care (ROC) and shall occasionally need to provide routine visits based on individual patient needs and Agency expectations.
- Ensures plan of care incorporates and guides appropriate teaching related to individual patient needs, health maintenance, prevention, infection control and safety.
- Coordinates available resources to manage care plans and patient care.
- Assures appropriate care of patients is met through the start of care assessment completion.
- Promotes/coordinates communication between team members, primary physicians, appropriate administrative staff, referral sources, and external case managers to ensure the appropriateness of care.
- Coordinates and communicates any necessary changes to the plan of care.
- Must complete head to toe assessment of patient and document all findings.
- Provides direct nursing skills under the direction of a NY Licensed Physican including med reconciliation. health teaching, health counseling, and patient care (i.e. infusion therapy, wound care, collection of laboratory tests, medication orders, medication administration, etc.).
- Provides health education specific to the needs of the patient and caregivers to enable them to reach their maximum level of independence and well being during the episode of care. This education takes into consideration their ability to learn, their cultural/spiritual background and their motivation to learn. Evaluates and documents the effectiveness of teaching methods and patient/caregiver response to teaching.
