Job Description:
Job Title/Position: Hospice Care LPN
Reports To: Director of Nursing / Hospice Administrator
Job Description Summary:
The LPN reports to the Director of Nursing and/or Hospice Administrator, and is to be clinically competent; deliver holistically, hospice focused care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.
Position Responsibilities/Standards:

Utilize the nursing process by demonstrating appropriate assessment of urgent needs, planning, implementation, and evaluation of the patient's care.

Assess the patient and document (same day) the patient's current health status in the Electronic Health Record. Make an accurate assessment of the patient's status and respond appropriately.

Plan/Implement: Set priorities for nursing action in a logical sequence according to patient's needs and revise the plan of care by the appropriate selection of nurse practice standards. The plan is based on patient/caregiver-centered goals and is agreed upon by the patient/Power of Attorney.

Evaluate outcomes related to situation/reason for care and revise plan of care based on new data and patient's condition/needs.

Document patient care which reflects focused nursing assessment, intervention, patient response to care provided, patient needs, problems, capabilities, and limitations. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions is noted in the medical record.

Communicate with the patient’s Attending Physician & specialists regarding patient needs, nursing assessment, and recommendations. Follow through with provider’s orders.

Collaborate with other interdisciplinary team members, as well as facility staff when appropriate to ensure the patient's safety and well-being.

Teach patient/caregiver appropriate health information in a timely manner. Document-level of understanding of education provided.

Perform direct patient care, utilizing established procedures, policies, and standards.

Administer medications according to policy and procedure.

Observe and document patients' responses to medications and treatments.
-
Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure.

Demonstrate necessary skills and knowledge to provide care for patients according to policy and procedure and based on palliative-specific competencies.

Demonstrate ability to handle emergency situations in a prompt, professional manner.

Provide personal patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.

Give a thorough, concise report with any changes in the patient’s condition or levels of care.

Communicate pertinent information to the appropriate person(s) in a timely manner.
-
Demonstrate accurate and timely documentation according to agency guidelines.
-
Accurately transcribe physician orders according to policy and procedure.
-
Maintain a clean and safe environment for patients & staff according to policy and procedure.
-
Demonstrate correct and safe techniques in the use of equipment according to specific product information.
-
Follow appropriate procedures for obtaining and returning or cleaning/disposing of equipment and supplies.
-
Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.

Remain conscientious in regards to personal hygiene and comply with dress code policy.

Comply with specific policies and procedures concerning safety.
-
100% attendance to mandatory skill updates yearly required.

Attend discipline-specific meetings and read and sign all minutes of meetings not attended.

Assist patients and family members with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.

Answer and refer phone calls or other inquires to ensure and timely communications are facilitated.

Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.

Position Specifications:
-
Must meet Desert Valley Hospice & Palliative Care requirements for employment.
Education:
-
Graduate of an accredited school of Nursing.
-
Valid registration as a Licensed Practical Nurse in the State of Arizona.
-
Current CPR card.
Experience:
-
Previous experience in hospice or palliative care is required.
-
Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
Physical Demands/Working Conditions:
-
Intermittent physical activity includes walking, standing, sitting, lifting, and supporting patients.
-
The position will be exposed to viruses, diseases, and infections from patients and specimens in the working environment.
-
The position will be required to work at any facility or patient’s home and be responsible for its own transportation.

The position will be scheduled based on operational needs.
