Job Summary
The Sepsis Program Coordinator position is one of educator, collaborator, consultant, and change agent. In collaboration with the medical and hospital staff and the Sepsis Quality Improvement team, the Sepsis program Coordinator maintains and implements Joint Commission (TJC) and CMS Core measure standards and evidence-based practices. This position is responsible for data collection and reporting, evaluating current state practices/performance against industry benchmarks and leads improvement efforts to achieve the highest quality care for sepsis patients.

- Conducts needs assessment, develops, and assists in the instruction of educational programs for nurses, physicians, and other healthcare team members; supports the Sepsis Quality Improvement Team.
- Networks with affiliate institutions, organizations/associations, and other health care providers to stay informed of developments and trends in sepsis care.
- Participates in establishing and modifying standards, policies, objectives, protocols and practice management guidelines for sepsis patient care consistent with best practices.
- Monitors the care of sepsis patients and serves as a resource to the healthcare team to ensure clinical progression toward defined quality outcomes.
- Develops and maintains appropriate and required clinical, administrative, performance improvement, patient safety and financial data analysis and reports.
- Abstracts, tracks, submits, and reports hospital sepsis Core Measure data to the quality department, Sepsis Quality Improvement Team, and national benchmarking agencies.
- Monitors and evaluates the performance of hospital departments and clinical services to recognize achievements and identify opportunities for improvement. Communicates the opportunities for improvement to the appropriate leader(s) and recommends and/or provides specific training programs to meet the identified needs.
- Serves as a leader, researcher, change agent to improve patient education opportunities.
- Establishes and cultivates respectful, collaborative relationships with stakeholders, colleagues, patients, and families to fosters exemplary customer service.

Job Qualifications
- Bachelor’s degree in nursing from an accredited college; Master’s degree preferred.
Experience
- 5 years minimum experience in nursing
- Experience in the care of the Sepsis patient population
License/Certifications
- Current California Board of Registered Nurse License
- Current American Heart Association BLS Certification
- Certified Professional in Healthcare Quality (CPHQ) preferred, within one year of role
Knowledge, Skills, and Abilities
- Ability to conduct chart reviews and abstract data from Epic/Midas.
- Ability to plan, organize, prioritize, work independently, and meet deadlines.
- Ability to manage multiple changing priorities and work effectively in a team or independent setting.
- Ability to communicate effectively at all organizational levels and in situations requiring instructing, persuading, negotiating, conflict resolution, consulting and advising.
- Excellent written communication and attention to detail.
- Ability to establish and maintain effective interpersonal relationships.
- Knowledgeable in the clinical management of the sepsis patient population within established guidelines.
- Ability to connect evidence-based best practices to system strategies to promote high quality, efficient, and cost-effective care.
- Project management
- Proficiency in the use of Microsoft Word, Excel, PowerPoint, and Outlook.
Job Type: Full-time
Pay: $133,000.00 - $180,000.00 per year
