Job Description
Harbor Care Foundation (HCF) is a non-profit organization that provides short-term residential care for homeless individuals who are recovering from an illness or injury whose condition would be worsened by living on the streets. We provide a safe haven for residents that are physically independent to recover and connect them to vital services and permanent supportive housing.
Summary
The primary role of a Care Monitor is to provide care to the residents at our recuperative care facility. Their duties include medication reminders, scheduling and transporting them to their follow up doctor appointments, obtaining prescription drug refills, performing standard care procedures, and checking vitals. They will also be responsible for general upkeep and cleaning of the facility.
Our Care Monitor will need to rely on their technical skills, so they understand how to use the equipment to take guest vital signs and software to chart medical information. They should also be detail oriented, as they must enter all resident data into our database so their department and others related departments can refer to them. Care Monitors should also have interpersonal skills so they can work with residents who may be experiencing mental health symptoms/diagnosis.
Essential Duties & Responsibilities
- Conduct intakes for our residents.
- Complete registration packets with our residents for clinic visits.
- Conduct rounds in our facility, obtaining vital signs, assisting with blood sugars checks and/or information as needed.
- Updating residents' charts daily using CRM database system, and scheduling appointments with them.
- Monitor medication management based on plan of care.
- Assist Care Monitor manager as needed.
- Coordinating and providing linkages for guest’s medical appointments and follow-up.
- Participating in weekly internal and external case conferencing, board review team meetings, monthly staff meetings, trainings, and in-services.
- Performing administrative tasks such as charting and answering phones.
- Coordinate transportation and sometimes transport residents for medical related matters.
- Completes additional tasks/duties as required to meet the needs of the facility, not to impede primary responsibilities.
- Always follow Harbor Care Foundation policies and procedures.
Qualifications
- Bilingual in Spanish (preferred).
- Ability to work independently with strong time management and organizational skills.
- Attention to detail and high accuracy are a must.
- Excellent verbal and written communication skills as well as a high level of professionalism required.
- Experience in medical office.
- Familiarity with HIPAA (preferred).
- Proficiency in Microsoft Suite products including Outlook, Word, Excel, Teams, etc.; and skill in using databases.
Physical Demands
This position requires extended time of work in an office environment working at a desk and time doing cleaning tasks to maintain the cleanliness of the facility. This position also may involve transportation of clients to various appointments as part of their recuperative care experience.
Work Environment
Although this position functions to a large degree in an office environment, it is in a respite facility with an at-risk population where errant behavior and risk of infectious disease may be present. Appropriate safety considerations will be required to avoid illness or injury.
Job Types: Full-time, Part-time
Pay: $18.00 - $23.00 per hour
