TITLE: Supervisor, Children Health Home Case Management Program
REPORTS TO: Program Director
Department: Children Health Home Case Management
RESPONSIBILITIES:
- Organize and supervise staff who support individuals with chronic mental/medical health illness and complex trauma in achieving short and long term health care goals using a person-centered model
- Minimum Caseload may be required should it be necessary to maintain operations.
- Reviews new enrollments and assigns to a care manager in a timely manner
- Manage staff in accordance with established policies and procedures and time frames.
- Ensure quality of the services delivered by the HHCM program.
- Ensure all CANS assessments and other mandatory assessments are completed
- Check critical clinical notifications on a daily basis
- Ensure the timely development and implementation of care plans
- Ensure that staff completes all necessary case notes and written reports in a timely, accurate and concise.
- Ensures that the needs identified in the Care Plan are delivered to the clients’ and guardian’s satisfaction.
- Works closely with his/her team and assist in identifying necessary resources to achieve client’s care management goals
- Ensures that client’s Medicaid and demographic information is accurate on a monthly basis
- Supervises the termination process and case closures and ensures that appropriate discharge planning is provided.
- Attend program related community and committee meetings as assigned
- Resolve issues based upon client complaints, provider concerns and/or data trends.
- Work collaboratively with intake and outreach staff, and Directors of all Interborough programs
- Other duties as assigned
REQUIREMENTS:
- Bachelor’s degree in a related field, a minimum of 2 years of Supervisory Care Management experience preferred
- Superior attention to detail, research, organizational and problem solving skills
- Ability to work independently, and as part of a team
- Ability to coordinate responsibilities. Excellent organizational and time management skills required.
- Background check and finger printing required
- Working knowledge of computer software and electronic health record systems.
- Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
- Excellent interpersonal skills required.
- Bilingual preferred: Yiddish, Spanish, Russian

SALARY RANGE: $65,000 to $70,000 (Based on experience and licensure/certification.)
