We are looking for a compassionate Registered Nurse to join our hospice team and family.
Position: Hospice Admission RN (Full-time or Part-time)
Shift: 11:00am - 7:30pm or 12pm - 8:30pm, 5 days a week, 8 hour shifts, alternating weekends
Reports to: Clinical Manager

The Hospice Admission Registered Nurse is the first member of our hospice team with whom patients come into contact and plays a pivotal role in the educational process for patients, families, and caregivers. You are the very first face they see as they embark on their journey.
As a Hospice Field Admission Nurse (RN), you will admit eligible patients onto the Hospice program following the appropriate standards of practice established by Medicare Guidelines. In this field position you will perform initial physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care, goals, and make recommendations based on each patient's needs. Assessments may take place in private homes, hospitals or nursing facilities.
Qualifications:
- Current California Registered Nurse (RN) licensure (in good standing) with one year of experience
- Provides compassionate education concerning hospice care philosophy, as well as the nature of the care they can expect to receive in regard to their specific terminal illness
- Guides patients, families, caregivers, community partners through the hospice assessment and admissions process.
- Work closely with Clinical Leadership, Physician, Intake Coordinator, and Liaison to understand the patient’s needs and determine patient's eligibility for hospice care.
- Utilizes strong clinical skills to evaluate hospice eligibility per Medicare guidelines.
- Ability to learn an electronic medical records system
- Must have personal transportation and be willing to drive within a 60 mile radius of our Corporate office.
- Must be available to work alternating weekends

Education
- Associate Degree in Nursing required.
- BSN is a plus.
