Over time our approach to mission has shifted in response to community need, for example, we have evolved from the early years as a day nursery for the children of working mothers, to after school programming with curricula which fosters good decision-making strategies for teens, to celebratory whole family events to promote parent / child connections. Case Manager, Young Parent Support:
This position will focus on providing in-home case management services to pregnant and parenting young adults, living in Chelsea and Revere, up to age 23. The goal of the Young Parent Support Program is to assist young parents in obtaining the necessary skills and knowledge to be competent parents, live independently, lead productive lives, and ensure the healthy growth and development of their children. The Case Manager will be tasked with the following responsibilities:
· Recruit and maintain a caseload of 15 pregnant or parenting young adults. Provide case management services, including, but not limited to assisted referrals, accompaniments, life skills training and interpreter support.
· Maintain a home visit, check in, and service delivery schedule with each participant based on contractual expectations. Complete timely intakes, assessments, service plans and case notes in order to ensure accurate delivery of service.
· Implement ASQ and ASQ/SE with children involved in the program.
· Create and maintain participant files manually and electronically according to LifeScene, Young Parent Support and the Department of Children and Families (DCF) standards.
· Complete required DCF and agency trainings.
· Develop and implement individual and group programming for pregnant and young parents.
· Provide participants with accurate and evidence-based information on pregnancy, childbirth, child development, sexual health and more. Communicate with collateral contacts as necessary and appropriate.
· Participate in and attend meetings activities, workshops, and special events as assigned.
· Engage in 1 hour of individual supervision with the Program Coordinator, Family Service Division and attend monthly all staff and program team meetings.
· Participate in community-based coalition meetings in Chelsea and Revere.
· Collaborate with community partners including but not limited to DCF, Chelsea and Revere public schools, Juvenile Probation, DYS, courts, hospitals, clinics, medical service providers, and others as appropriate to address, support and resolve barriers i with participants.
· Assist with emergency and crisis intervention.
· Other duties as assigned.
Qualifications and Preferred Skills:
· A bachelor’s or associate degree, preferably in human services, with a minimum of 2-5 five years’ experience working with families.
· Ability to demonstrate extensive knowledge of community resources in Chelsea and Revere.
· Ability to establish trusting relationships, and to think and act intentionally and strategically to help participants.
· Capacity for creative problem-solving, conflict resolution, ability to work as part of a team, with strong verbal and written communication skills.
· Must be able to travel with appropriately insured personal vehicle and maintain a valid MA Driver’s License.
· Be computer literate and detail oriented.
· Experience working with diverse populations.
· Have passion and be committed to directly collaborating with young parents and their children.
· Must be Bilingual (English/Spanish)
