Summary
The Liv Well Family Care Coordinator is responsible for ensuring the clients receive the care that they need. They collaborate with multiple departments to help determine what type of treatment and goals that the client will have. The Family Care Coordinator will also track the client's progress and communicate with family members relating to the client's care. They will check in with clients and help determine if adjustments to their care are needed.
Primary Responsibilities
- Complete initial client onboarding and paperwork

Communicate with parents weekly on the progress and effectiveness of treatment. Also, learning of any changes that occur at client's home that the team would need to be aware of

Staff new clients with the BHT team

Debrief with BHT team to understand how clients work towards their goals are going

Updating treatment plans and assessments every six months, or as needed, to make sure goals are seeing progress or being adjusted for appropriateness of services

Meeting with every family on caseload at minimum monthly for a family check

Assisting families to accomplish tasks that require guidance or extra help (AHCCCS appointments, crisis intervention, etc.)

Filling for direct care when needed or helping the direct care team in some capacity during school breaks
Requirements
- Must have a valid driver's license and safe driving record.

Ability to maintain composure under stressful conditions.

Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.

Time management and prioritization skills to ensure efficient functioning of schedules and office systems.
Schedule

Monday-Thursday, 1pm-9pm

Friday, 8am-4pm
School Breaks (Summer, Winter, etc.)
Education & Experience
- High school diploma or equivalent

Experience working with kids preferred

Six (6) months of experience in a healthcare setting OR completion of a related health education certification OR completion of the first semester of courses in a major of an associate or bachelor's degree program in psychology, nursing, counseling, or related field.
