Responsibilities:
- Provide compassionate and comprehensive care to patients in hospice settings
- Conduct assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and manage symptoms to ensure patient comfort
- Collaborate with interdisciplinary team members to coordinate care
- Educate patients and their families on disease management and end-of-life care options
- Assist with discharge planning and coordination of services
- Document patient care activities accurately and timely
Qualifications:
- Valid nursing license (RN or LPN) in the state of practice
- Minimum of 1 year of nursing experience, preferably in hospice or palliative care
- Knowledge of hospice regulations and standards of practice
- Excellent communication and interpersonal skills
- Ability to provide emotional support to patients and families during difficult times
- Strong critical thinking and problem-solving abilities
Note: Additional certifications or experience in specific areas such as spinal tap, mammography, special needs, airway management, sterile processing, ostomy care, esthetic laser treatment, discharge planning, coding, or other specialized skills may be preferred or required depending on the specific position.
