The Licensed Vocational Nurse’s primary function is to administer skilled nursing care, under supervision of a registered nurse, for patients/clients of all ages in their places of residence. Participates in the coordination of care with the interdisciplinary team, patient/client, family and referring agency.
QUALIFICATIONS:
- Graduate of an accredited School of Vocational/Practical Nursing.
- Current State license as a Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
- Current Driver’s License
- One-year experience as Licensed Vocational;/Practical Nurse in a clinical care setting required. Home Health preferred.
- Reliable Transportation. Valid and Current auto liability insurance.
Essential Functions:
- Under the direction of RN, assist in identifying the patient's/client's physical, psychosocial, and environmental needs as evidenced by documentation.
- Participates in planning, implementing care, and education in partnership with the patient/client, representatives (if any), caregiver(s) and with the RN.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Assures continuity of care.
- Monitors assigned cases to ensure compliance.
- Participates in the Agency's QAPI program.
- Participates in Agency-sponsored in-service training.
- Demonstrates commitment, professional growth and competency.
