THE PROGRAM
The Behavioral Health Link program is a one-stop shop for resources for the homeless. Many organizations are coming together under one giant resource tent to provide multiple services to stabilize active health conditions and to coordinate linkage with other psychosocial and housing resources.
THE POSITION
Seeking a licensed Advanced Practice Nurse Practitioner, with interest and experience in mental health and substance abuse disorders, to participate with team of mental health therapists, drug counselors and support staff to coordinate short term care and effect linkages to ongoing services in the community. The Nurse Practitioner will be the lead medical staff in the project and will consult with the team and with other agency staff in determining the mental health and substance use disorder treatment needs of individuals residing in the facility. Duties will include performing triage diagnostic assessments for referred individuals to include client interviews, review of accessible health records, documenting findings and interventions, and collaborating with the other professionals on site and in the community.
The Nurse Practitioner will prescribe short term medication—refills or replacement medications for individuals with immediate needs and will work with other facility staff in effecting timely referrals to community resources able to provide ongoing care to clients. An active full scope license and furnishing credential within the state of California is required. The Nurse Practitioner will work under the supervision of a senior board-certified psychiatrist with access to a team of consulting psychiatrists.
Priority Population: Persons served will be Transition Age Youth (TAY) and Adults & Older Adults (AOA) age 18 or older, with mental health and/or substance use conditions or with co-occurring conditions who are voluntarily residing at The Nexus campus with lengths of stay of upwards of 90 days. The Behavioral Health Link campus population will include diverse homeless populations defined as including racial and ethnic groups in the Central Region of San Diego County (Midway area of Point Loma). Individuals served will include both women and men, veterans, older teenagers (18+) and older adults. Many individuals will have co-occurring physical health conditions, and may present with significant trauma and come from the full array of marginalized communities.
The Nurse Practitioner will assist the license clinicians in the program in planning and implementing care coordination services to connect participants to behavioral health services, physical health services and community resources. As needed, substance using individuals and those with mental health disorders will be referred to off-site treatment services, including but not limited to outpatient and residential treatment, Medication Assisted Treatment (MAT) and syringe service program.
REQUIREMENTS
· Graduation from an accredited Nurse Practitioner program, with relevant board- certification and an active unrestricted license to practice as an NP in California.
· Current DEA Number.
· Knowledge and experience in providing psychiatric and addiction related care to people from diverse cultural backgrounds
· Ability to pass criminal/background clearance, TB, & drug screen requirements.
· Bilingual Spanish preferred, but not required
HOURS
Generally 6 hours per week as needed. Hours are flexible between 8am-8pm, Monday through Friday. Service delivery will require both on-site and televideo capabilities.
PAY
$80.00 per hour
This is a starting rate and may be adjusted up based on education and experience.
