We are currently seeking a Certified Medical Assistant to serve as Clinic Care Coordinator for one of our Participating Clinics located in Mercedes, TX. This clinic-based, full-time position requires a minimum of 1-3 years experience within physicians’ practice, hospice or any other related medical setting. Experience in case management, care coordination, transitional care management, and population health management are preferred but not required.
Primary Responsibilities:
- Coordinates timely patient care to avoid unnecessary hospital admissions and readmissions
- Helps providers with the management of quality measures
- Participates as liaison among entities involved with the patient
- Assists in the coordination of transitions of care and medical interventions
- Works as a patient advocate to support and enable access to community resources
- Assists in the coordination of hospital admissions and discharges, and other required follow-ups
- Collaborates with patients, caregivers, families, and community agencies to include all applicable levels of care
- Point of contact to receive and assess in a timely manner, hospital admissions and ER notifications from the ACO’s CC Department or other hospital portals
- Demonstrates and promotes a culture of value-based, patient-centered care
- Promotes an outstanding patient experience in the clinic
- Utilize electronic health record systems to document patient information accurately
Skills:
- Bilingual (English/Spanish) required;
- Strong communication skills to effectively interact with patients, families, and healthcare team members

- $10-16/ hour depending on level of experience and background;
