Description:
Evaluates and treats patients using the principles and practices of physical therapy and in accordance with the patient’s age, developmental level, and diagnosis. Collaborates with the interdisciplinary team. License-eligible therapists would perform duties under the supervision of a licensed therapist in accordance with the Illinois Physical Therapy Practice Act.
Provide "One on One" treatment to provide each patient with the highest level of care.
Opportunity is for full time - Monday- Friday, 8am-4:30PM, rotating weekends/Holidays. 5k Hire On Bonus
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Evaluate Out Patients, establishes goals, and develops individualized treatment plans as prescribed.
2. Provides direct patient treatment plan based on the patient's response.
3. Provides education to patient regarding patient’s abilities and assistance for therapeutic intervention.
4. Recommends modification or changes in the patient's treatment program to the referring physician.
5. Prepares written/computer documentation as required by the profession and the department.
6. Participates in the interdisciplinary patient care conferences and rounds to ensure health care plan.
7. Communicates with other professional staff and patient's family/support system to provide continuity of care.
8. Arranges work schedule to make efficient use of direct and non-direct patient care time.

Education and/or Experience

1+ years of experience in Outpatient highly desirable

A Bachelor’s degree is required

A Master’s and/or Doctorate Degree in Physical Therapy is preferred

EMR/EPIC experience preferred

Microsoft Office (Word and Excel) experience preferred

Basic computer and typing skills required

Current Illinois license in Physical Therapy
