PURPOSE OF JOB:
Provide quality diabetes care and education to the person with diabetes in accordance with the National Standards. Provides education to the community including outpatient classes and follow up. Educates/certifies nursing staff for use of bedside glucose monitor.
ESSENTIAL FUNCTIONS:
1. Provides initial and follow-up diabetes care for inpatients and outpatients with diabetes according to hospital and National Standards, with consideration to age-specific accommodations.
2. Communicates with patients, families, physicians and other health care team members in achieving excellence in diabetes care and education.
3. Provides bedside glucose monitoring training for staff.
4. Actively participates in and supports departmental and organizational quality initiatives and goals.
5. Maintains clinical competency.
QUALIFICATIONS:
Education:
1. Current RN licensure by the State Board of Nursing, Commonwealth of Pennsylvania
2. Graduate of an accredited School of Nursing, BSN preferred.
3. Certified Diabetes Care and Education Specialist.
Experience:
1. Three to four years clinical experience
2. Teaching experience, both formal classroom and informal.
Other Skills:
1. Excellent interpersonal skills
2. Certified Accuchek Inform Operator, Accuchek Inform trainer preferred.
