For over 70 years, UCP has provided supports and services to individuals with cerebral palsy and other physical, developmental and intellectual disabilities and issues of aging, promoting the full inclusion in all aspects of society.
UCP’s Residential Program is in search of three (3) floating LPN/ Care Coordinators for our south regions residential programs (2) and for our Boston region (1). We are seeking caring professionals with strong communication and advocacy skills. The LPN would work directly under the Cluster Director and Registered Nurse Health Care Coordinators in medical and ABI homes.
Responsibilities: The responsibilities of the LPN include supporting all aspects of medical and behavioral needs of the individuals in the program, but not limited to:
- Accompanying individuals to medical appointments and procedures.
- Assisting the RN in developing annual nursing assessments, as well as quarterly and monthly medical reviews.
- Providing direct care support to the individuals in relation to activities of daily living (ADLs)
- Providing skilled nursing responsibilities such as vital sign checks, catheter exchanges, insulin injections, and G-tube care.
- Medication administration and monitoring.
- Some training responsibilities; to supervise and to provide support to direct care staff pertaining to the medical needs of the individuals and general health and household needs.
- Actively participating in clinical and other staff meetings with members of the multi-disciplinary team.
Qualifications: Currently registered with Massachusetts Board of Registration in Nursing. Must be certified by an accredited nursing program and have some experience working with individuals with developmental disabilities and physical disabilities. Candidates must have current Basic Life Saving (BLS) certificate. Must have a valid driver’s license. Basic computing and reporting skills required. LPN must be familiar with the State’s Regulations related to the Medication Administration Program (MAP).
Working Conditions: The LPN is expected to commit the time and talents necessary to fulfill the requirements of the position in an exemplary manner. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Traveling is required and is primarily local to and from locations including transporting individuals in large or small vehicles. The LPN must possess the physical ability and willingness to assist persons served in personal care, which will include physical transfers and assistance with bathroom routines. Must have a full range of motion including the ability to bend, push, pull, bear-weight, and stoop for approximately forty-five minutes at a time.
