The Maternal Educator is responsible for assisting with the development and implementation of a program focused on providing maternal patients with resources and support in an effort to achieve improved care outcomes and meet established quality measures goals. This individual will support the provision of a positive and educational experience for maternal patients through outreach and care coordination with other departments.
- Ensure that the needs of maternal and newborn patients are met by coordinating care through follow up, appointment scheduling and outreach.
- Collaborate with contracted Obstetrics & Gynecology (OB/GYN) Providers to assist with facilitating support and resources to patients throughout prenatal and postpartum care.
- Conduct outreach to pregnant patients and offer assistance, support, and resources for prenatal and postpartum care.
- Conduct outreach to postpartum patients and offer assistance , support and resources for the care of newborn child.
- Assist members with selecting a Desert Oasis Healthcare Pediatrician for their child/children and following up to ensure visits are scheduled and kept to ensure Quality Measures for both the patient and their baby/babies are being met. Offer assistance with selecting a contracted Pediatrician.
- Compile relevant educational material from local groups and/or qualifications to disseminate to patients as deemed appropriate. Create educational material and distribute to patients as deemed appropriate.
- Collaborate with other departments to facilitate patient referrals, appointments and other needs.
- Develop, implement and maintain a lactation group focused on supporting a positive breastfeeding experience for newborns and nursing mothers.
- Track, monitor and compile data on outreach efforts and achieved quality measures goals relevant to maternal and newborn patients. Report findings to appropriate management and staff.
