Job Description:
Fulton Family Psychiatry, LLC (FFP) is seeking a dedicated and licensed Clinical Social Worker to join our dynamic team. As an LCSW, you will play a crucial role in providing comprehensive mental health services to our diverse patient population. This position requires a candidate with a strong background in social work, a passion for holistic care, and experience in substance abuse programming is a plus.
Responsibilities:
- Clinical Intake: Conduct comprehensive clinical intakes and assessments to evaluate patients' mental health needs and develop individualized treatment plans.
- Facilitating the Distribution of Diagnostic Surveys: This process involves the organized distribution of diagnostic surveys to collect essential data on the mental health status and progress of treatment in patients. These insights are invaluable for potential research studies focused on the outcomes of employing integrative psychiatry approaches.
- School Liaison: Act as a representative of FFP during school meetings to advocate for our patients' needs and coordinate mental health services within the school system.
- Substance Abuse Program Development: Work in partnership with our team to create and execute an innovative, holistic substance abuse program. Utilize your specialized knowledge in addiction treatment and recovery to achieve OASAS certification, making the program accessible to everyone by accepting both private and public insurance at Fulton Family Psychiatry.
- Patient Coordination/Case Management: Act as a liaison among patients at Fulton Family Psychiatry, educational institutions, and Primary Care Providers (PCPs), facilitating integrated communication and comprehensive care.
Requirements:

- LCSW License: Must hold a valid LCSW license in the state of New York
- Experience in Addiction Treatment: Prior experience or specialized training in substance abuse treatment and recovery is highly desirable.
- Strong Communication Skills: Excellent interpersonal and communication skills are essential for collaborating with patients, school officials, and healthcare professionals.
- Team Player: Ability to work collaboratively within a multidisciplinary team environment to provide comprehensive care to patients.
