We are seeking Medical Assistants (bilingual MAs are a plus!) to join our team as Outreach Coordinators ( OC’s)!
This is a very important role in our organization, because, as an OC, you will be a patient’s first point of contact with the Wellacy Health team, so you will be responsible for delivering high quality, compassionate program education and obtaining patient consent for participation.
Responsibilities:
- Deliver high quality, compassionate services to our patients and prospective patients by making up to 50 outbound calls per day to enroll a minimum of 20 patients per week
- Educate patients about chronic care management services, including benefits, eligibility criteria, and program requirements
- Obtain patient consent for enrollment in chronic care management programs and ensure all necessary documentation is completed accurately
- Collaborate with healthcare providers to identify eligible patients for chronic care management and obtain relevant medical history and information
- Maintain accurate and up-to-date records of patient enrollment, consent forms and communications
- Scheduling patient appointments with care coordinators
- Managing inbound patient support calls and routing them to the appropriate clinical team members
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive care coordination and continuity for patients enrolled in chronic care management programs
- Enrollment data management
- Stay informed about changes in healthcare regulations and guidelines related to chronic care management and ensure compliance with all relevant policies and procedures
- Successfully managing enrollment workflows
Qualifications:
- Current, Valid Medical Assistant registration
- Experience working in a healthcare environment (at least 12 months)
- Demonstrates creative solution-based problem solving, critical thinking skills, and sound judgment
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Strong communication and interpersonal skills, with the ability to effectively educate and engage patients
- Highly organized with the ability to manage time and prioritize workflow
- Proficiency in using electronic health record systems and other healthcare software applications
- Knowledge of medical terminology, chronic conditions, and healthcare regulations related to chronic care management.
- Ability to be self-directed and work autonomously
- A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust
- Maintaining knowledge and awareness of organizational standards for an outreach coordinator is required
