Mt. San Antonio Gardens is currently seeking per diem Registered Nurses to join our dedicated team of nurses offering Skilled Nursing Care to our residents. The ideal candidate is passionate, experienced, energetic, and dedicated to delivering exceptional nursing services to our residents daily. The Registered Nurse will be responsible for directing all nursing staff and monitoring all patient care during his/her tour of duty and is responsible for implementing facility policies and procedures; coordinating nursing activities with other services provided by members of the health care team.

Responsibilities:
- Supervises and evaluates the performance of all licensed personnel.
- Reviews skilled nursing facility residents' charts to assure accurate and complete charting.
- Attends meetings necessary to provide continuity of care for residents.
- Assists with coordination and completion of the MDS process; encompassing admission and change of condition audits.
- Assists with the care plan meetings, quarterly MDS care plan QA study.
- Assists Social Services Designee with discharge planning for all skilled nursing facility residents.
- Coordinates with medical records for audits to comply with policies and regulations.
- Cooperates with Director of Staff Development to implement in-service education and to verify staff participation as required by law.
- Works with purchasing to assure that equipment/supplies are provided.
- Acts as Director of Nursing Services in the absence of the Director of Nursing Services and the MDS/Resident Care Coordinator.
- Attends mandatory licensed staff meetings.
- Assesses resident needs and initiates development of individualized plans of resident care. Reviews and revises resident care plan as necessary and as required by regulations.
- Performs basic nursing skills, demonstrating knowledge and competence.
- Reports changes in resident's condition to a physician, Director of Nursing Services and responsible party promptly and takes follow-up action as necessary.
- Accurately, completely and timely documents resident care provided and residents' response or lack of response to care provided.
- Administers medications and treatment per the physician's order and accurately records all care provided.
- Practices universal precautions.
- Assist in responding to call lights as needed regardless of assigned residents within 3 minutes.
- Assists with basic nursing care as needed.
- Practices good safety habits.
- Assumes MDS Coordinator position when DONS and Assistant MDS Coordinator is not available.
- Completes data entries into Quick Care Clinical System in a timely manner.
- Completes other duties as assigned by the Director of Nursing Services.
- Possession of a valid California Registered Nurse license.
- One (1) year experience in long term care or geriatrics.
- Certified in IV therapy.
- Ability to maintain effective working relationships with patients, doctors, employees and volunteers.
- Knowledge of modern nursing methods and care.
- A working knowledge of medical records.
- Knowledge and ability to use the computer correctly.
