Job Summary:
Care Coordination services for youth enrolled in Foster Care under Article 29-I. Maintain and ensure compliance of Health Service records for youth care. Work in collaboration between Health Services and other departments within the organization. Develop and sustain working relationships with community providers to ensure continuity of care for youth in care.
Duties and Responsibilities:
- Identify health needs of youth in care and their caretakers through direct or indirect means of interaction.
- Act as a liaison between multiple providers in the provision of health services for our youth in care.
- Establish a compassionate environment by providing support to youth and their families.
- Promote youth’s independence by establishing goals, teaching clients, families and caretakers to understand conditions, medications and self-care skills.
- Resolve youth medical problems and needs by utilizing multidisciplinary team strategies.
- Document all related correspondence related to client care needs. Daily update of relevant medical records in accordance with compliance measures from both stakeholders land the agency alike
- Communicate with youth, their families and multidisciplinary team by providing client related health information on a routine basis (minimum monthly outreach) this includes but not limited to: information sharing, responding to requests for service updates or needs, and being an active participant in the client centered problem-solving methods.
- Utilizing a Trauma Informed approach and framework into daily practice to ensure quality care and an effective work environment.
Skills, Knowledge, and Abilities:
- Excellent verbal and communication skills with keen attention to detail.
- Ability to work as part of a collaborative team.
- Strong computer skills including Microsoft, Excel, and Connections.
- Must have high level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy.
- Ability to work in a fast-paced environment with demonstrated ability to juggle and prioritize multiple, competing tasks and demands and to seek supervisory assistance as appropriate.

Qualifications/Requirements:
- NYS LPN required. HS diploma/GED required. Associates preferred.
- Case management experience preferred.
- Due to the nature of the work environment work hours will be based on needs of program and may vary.
- Performs other duties/responsibilities as assigned within scope of position.
- Our Agency operates in 2 locations, Staten Island and Brooklyn. The ability to travel between sites and perform assigned tasks and responsibilities may be necessary.

- Compensation range of $30.00 to $31.48 per hour based on the candidate’s education and experience.
- Standard work schedule of 9am to 4:45pm Monday trough Friday with 3 days in office and 2 days remotely.
