Garfield County Collaborative Management Program Coordinator
Definition:
The Collaborative Management Program (CMP) aims to help families and young people aged 0-21 in Garfield County who are involved with multiple agencies. The program is overseen by the Interagency Oversight Group (IOG). Local schools and youth-serving agencies refer families to the program, and the Family Resource Center of the Roaring Fork Schools, in partnership with Garfield Re-2 and Garfield 16, screens them to determine which community services are appropriate for them. The program includes interagency family meetings of Individual Service and Support Teams (ISST). The GarCO CMP Coordinator will provide programmatic support to the three Family Resource Centers in RFSD, RE2, and Garfield 16. They will collaborate with eligible families who have children with mental or behavioral health challenges, and academic or developmental concerns and are involved with multiple systems.
Supervision Received and Exercised:
Director of the Family Resource Center of the Roaring Fork Schools: Provides oversight, training, coaching, and support to the coordinator.
Primary Duties:
- Establish and maintain critical relationships to enhance program awareness and financial sustainability.
- Primarily responsible for coordinating the local Collaborative Management Program activities for Garfield County.
- Develops and maintains strong, positive relationships with IOG/CMP partners through effective coordination, communication, and continuity.
- Manages the Garfield County Collaborative Management Program and ensures compliance with state contractual responsibilities.
- This includes participation in community systems-building initiatives, reducing duplication of services, and eliminating fragmented services to families.
- The position also facilitates Individualized Services and Support Team (ISST) family meetings and assists in developing an action plan.
- Will provide family development services and resource management (case management) to families experiencing complex needs for 12 weeks.
- Organize, coordinate, and support the IOG members, IOG meetings, and CMP processes as outlined in the Bylaws.
- Participate as a mandated partner in Garfield, Eagle, and Pitkin Collaborative Management Programs (CMP) Individualized Service Student Support meetings and IOG meetings.
- Initiate and maintain ongoing communication with IOG members.
- Report outcomes and provide program updates monthly to IOG members.
- Establish positive working relationships with community resources, service providers, and referral sources.
- Maintain communication with partners, contracts, and reporting.
- Collaborate with the community to understand and respond to community needs.
- Remain informed regarding best practices in youth and family programs.
- Initiate and promote positive relationships with community and state resources.
- Maintain administrative responsibilities.
- Write and distribute IOG meeting agendas and minutes.
- Provide CMP orientation/education to IOG members.
- Provide CMP orientation/education to referral sources.
- Identify and support IOG subcommittees (financial and program).
- Maintain program website.
- Monitor program budget in collaboration with the fiscal manager.
- Process invoices for programs in collaboration with FRC leadership/fiscal manager.
- Adhere to FRC of the RFSD funding guidelines and protocols.
- Ensure accurate and timely required data entry.
- Track and monitor program numbers and outcomes.
- Attend state CMP State Steering Committees and Coordinator meetings as scheduled.
- Complete the annual report for the CDHS executive director.
- Enter all data into ETO for the annual report.
- Participate in professional development opportunities.
- Program Support
- Work with the team to screen and place incoming referrals in appropriate programs.
- Identify and support existing ISST meetings at schools, the Department of Human Services, and other community partners.
- Develop and maintain the ISST meeting tracking system.
- Provide training to community partners for ISST meeting facilitation.
- Schedule and facilitate ISST meetings as necessary for the RFSD Family Resource Center.
- Communicate with referral sources to determine youth program eligibility.
- Support ISST meetings in the identification of case managers and the development and distribution of collaborative service plans.
- Manage or delegate management of ISST cases, encouraging collaborative case management.
- Monitor the satisfaction of families through evaluation and participant surveys.
- Other tasks as assigned
QUALIFICATIONS
Knowledge of:
Knowledge of Garfield County population and resources.
Working within an educational community with parents and staff.
State and federal legislation regarding children, families, health, and human services.
Community programs and resources for families and students.
Child Welfare and Family Development Practices.
Trauma-Informed and Restorative Justice Practices.
Principles of supervision and training and performance evaluations.
English usage, spelling, grammar and punctuation.
Modern office procedures and methods.
Advanced knowledge of Microsoft Office programs, especially Word and Excel.
Knowledge of database concepts.
Principles and procedures of record keeping.
Principles of business and letter writing and basic report preparation.
Ability to:
Bilingual, bi-literate, and bicultural in Spanish and English required.
Oversee and coordinate the work of team members and partners

Monitor and track budget expenditures according to RFSD processes.
Facilitate communication with Spanish-speaking families or find appropriate interpretation/translation.
Prepare clear and concise administrative and statistical reports
Respond to requests and inquiries from building administrators, vendors, and the general public.
Develop, interpret, and apply RFSD policies and procedures.
Make public presentations.
Work with Boards and Commissions.
Recognize potentially hostile situations and diffuse situations appropriately.
Operated and used modern office equipment, including a computer and various software packages.
Communicate clearly and concisely, both orally and in writing.
Establish and maintain effective working relationships with those contacted during work.

Experience:
- Two years of experience working with school-age children and their families.
- High school diploma or equivalent. Supplemental education is preferred.
- Ability to obtain and maintain a valid driver’s license.
- Two or more years of experience working with linguistically and culturally diverse families.
Training & Education
A bachelor’s degree or equivalent from an accredited college or university with major coursework in Health and Human Services, Public Health, Business Management, Mental Health, and/or Education.
License:
Ability to obtain and maintain a valid Colorado Drivers’ License.
LOCATION AND WORKING CONDITIONS
Location: The position will primarily be at the Carbondale District Office.
Working Conditions: While performing the duties of this job, the employee regularly works inside. Office environment with exposure to computer screens and use of a keyboard. The
The employee is frequently required to sit, walk, and stand. There is occasional lifting of 25 pounds or less. The noise level is usually moderate, with many interruptions.
SALARY RANGE AND DAYS WORKED
Salary: This position is funded by the Family Resource Center of the RFSD, an independent budget based on blended funding. The salary range is: $50,000 - $60,000, depending on experience.
Days Worked: 252 days/year (with paid personal time, paid sick days, and paid time off for holidays and district non-work days such as during winter and spring break). Most building principals may require front office staff to work additional days before the beginning and end of the school year.
