New York Independent Assessor Program
Is seeking board certified Nurse Practitioners FNP, ANP, AGNP, - to support the New York statewide NYIA Independent Practitioner Panel that issues physician orders for individuals who have undergone a Community Health Assessment and are eligible for community-based long-term care services and supports. The NP will follow up with individuals who have received an assessment.
Coverage Areas needed: Brooklyn, Manhattan
- $115.00 per hour – community health in-home assessments
- Must be board certified and carry personal medical malpractice.
- Must have an active NYS Medicaid ID number.
- Must be conflict free; cannot have a Conflict of Interest (COI) - including working for an MLTC, LHCSA, Certified Home Health Agency, Insurance or ISNP
- Must have at least 2 years (3600 hours) of prior experience in any setting to qualify.
- Schedules available Monday – Friday 8am – 7pm; Saturdays and Sundays 10-6.
- Laptops are provided by the client.
- Training is virtual classroom, instructor-led 15 hours (paid).
- 1099 role
JOB RESPONSIBILITIES
- Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
- The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, in order to assist the Physician in the completion of a Physician’s order.
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
- Work collaboratively to monitor and address clinical quality and gaps in care
