As a premier behavioral health facility in Holyoke Massachusetts, we are seeking motivated, reliable, and committed caregivers who will be responsible for providing direct care and support to our patients within a substance use disorder program.
How You Care for Our Patients
As a Clinician II:
- Maintain an active caseload providing individual, family, and group counseling along with case management. Frequency of sessions based on clinical need and assigned levels of care.
- Complete biopsychosocial assessments, care plans, case management, care plans, periodic assessments, progress notes, discharge summaries and other related documentation as required within established timeframes.
- Track patient adherence with program requirements.
- Be a role model in professional relationships and boundaries with patients, colleagues and the community.
- Attend all department meetings, supervision and staff education/training sessions.
- Assist with insurance utilization review.
- Other duties as assigned.

You Bring Meaningful Experience
Successful candidates will have the following:
- Master’s degree in social work, psychology, mental health counseling, human services or related clinical field.
- Prior experience working within an outpatient behavioral health/substance use setting, including day treatment programming preferred.
- Advanced level knowledge and experience in the application of clinical behavioral health treatment and family system modalities for patients with substance use disorder, as well as risk and safety assessments.
- Sufficient typing and computer skills to facilitate use of Electronic Medical Records (EMR) required; experience with EMR, including SMART preferred.
- Possess an understanding of appropriate community supports and systems available to patients after discharge. Demonstrate competency and possess the knowledge base, skill and ability to work in an outpatient setting.
- New grads welcome to apply with previous outpatient or crisis experience.
