We are currently seeking Medical Social Worker staff for immediate full-time opportunities within our inpatient Medical Surgical and Float units. As a member of our inpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understand the impact of their diagnosis, and coordinate complex hospital discharges. Additional duties will include:
DUTIES
- Collaborating with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs.
- Acting as an advocate for patients to resolve crises and provide appropriate interventions.
- Investigating suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary.
- Completing biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
- Referring patients to community or social service programs to meet their complex needs.
REQUIREMENTS
- Master’s degree in social work
- New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) - REQUIRED
- Hospital/Medical experience preferred but new grads are welcome.
- Knowledge of medical terminology
- Understanding the roles of the healthcare team
- Exceptional interpersonal skills
- Deep understanding of the psychosocial aspects of illness and health
- Knowledge of community-based organizations and resources for discharge planning
- Superb written and oral communication skills
- Strong crisis intervention and short-term counseling skills
- Excellent time management skills and ability to work in a fast-paced work environment.
- Must be able to work Weekend and Holiday coverage on a rotating basis.
SALARY: $36.04/hr. - $48.74/hr. DOE Plus PTO & Full 1199 Union Benefits
