Mental Health Clinician - ACT/FACT
Join our client, a prestigious nonprofit organization in New York City in making the difference in the lives of individuals in need. We are seeking a compassionate and committed Mental Health Clinician to join the team. Openings in Multiple Boroughs, NYC
Key Responsibilities:
- Collaborate with a multidisciplinary team to deliver services consistent with the ACT model
- Participate in daily team meetings to review and plan client services
- Provide a specified number of monthly services to clients, with 80% delivered in the community
- Ensure timely and high-quality completion of required documentation
- Work closely with clients, their families, and community partners to support treatment and recovery goals
- Coordinate client care by liaising with hospitals, substance use treatment facilities, and other service providers
- Offer 24-hour crisis intervention services on a rotating basis
- Conduct immediate need assessments, comprehensive psychosocial assessments, and crisis prevention plans
- Perform specialized clinical assessments for violence risk, suicide risk, substance abuse, trauma, and criminogenic needs
- Utilize evidence-based treatments such as cognitive-behavioral therapy, harm reduction, motivational interviewing, and trauma-informed care
- Provide services to address client needs such as family support, housing, entitlements, employment, education, vocational interests, and community integration
- Assist clients with legal advocacy, navigating the criminal justice system, and coordinating with legal agencies
Qualifications:
- Master's degree in social work or mental health counseling, LMSW, LMHC-LP or CASAC certification (Supervision for licensure or limited permit holders can be provided)
- At least two years of experience working with adults with serious mental illness
- Must have or be eligible to obtain a New York State ID non-driver's or driver's license
- Proficiency in electronic health records
- Fluency in Spanish is highly desirable
