Performs direct patient care based on the plan of care while ensuring age appropriate care. Provides care for patients by providing assistance with activities of daily living: bathing, shaving, mouth care, skin care, toileting/incontinence care, repositioning to prevent breakdown for the bedbound patient, safety and monitoring with ambulation, progressive mobility and safe patient handling. Assists with incidental activities of daily living: light housekeeping, meal preparation, laundry, errands, etc. Keeps licensed staff informed of all responsibilities on an on-going bases, ensuring prompt notification in the event of a problem or potential problem or safety concern. Considers each patients physical limitations and developmental age providing special attention or special accommodations. Delivers care that is culturally sensitive. Adhere to agency policy for documentation.
