A licensed medical social worker will provide social work services under the direction of a physician. Social work services will be based on the patient’s psychosocial assessment and the patient’s and family’s needs. The social worker will coordinate care and participate in on-call rotations per the needs of the hospice. Guiding Light Hospice is seeking a full-time licensed medical social worker in San Antonio and the surrounding areas. Hospice experience preferred. As a Licensed Medical Social Worker, you will be expected to:
- Assess and complete the Comprehensive Assessment within five (5) days of the patient’s election to hospice care to identify the patient and family’s psychosocial, financial, environmental, and community needs as evidenced by POC documentation, clinical records, and community resource referrals.
- Assist with advance directives, funeral planning, discharge planning, and family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.
- Use effective interpersonal communication skills, both verbal and written.
- Meet mandatory continuing education requirements of the Hospice and licensing board.
- Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
- Provide patient and family/caregiver counseling around issues of end-of-life, death, and grief.
- Promote Guiding Light Hospice philosophy and administrative policies to ensure the quality of care.
- Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
- Provide an environment that promotes respect for patients and their privacy and property.
- Participate in the Guiding Light's Quality Assurance Process Improvement (QAPI) program.
- Demonstrate commitment, professional growth, and competency. Follows the Hospice infection control policy.
- Participate in the Guiding Light’s educational in-service programs as required.

Qualifications
- Social Work (LMSW) degree from a school of social work accredited by the Council on Social Work Education; from an institution accredited by the Council on Social Work Education; and is supervised by an LMSW; and has 1 (one) year of social work experience in a healthcare setting, preferably Hospice agency.
- Licensed as required per Texas laws.
- Current driver's license
- Possess reliable transportation with current vehicle insurance.
- Willingness to travel
- One (1) year of experience in palliative care or hospice preferred.
- Working knowledge of community resources. Good interpersonal skills. Demonstrates ability to work with computers.
- Bilingual (English/Spanish) a plus.

- Works in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to t travel locally; exposure to unpleasant weather.
Physical and Mental Effort
- Prolonged standing and walking are required. It may require occasional work under stressful conditions, providing counseling and education to terminally ill patients and their families.
- Ability to lift to 50 pounds and move patients. Requires working under some stressful conditions to meet deadlines, identify patient needs, make quick decisions, and meet patient and family needs. Requires eye-hand coordination and manual dexterity.
Pay: $50.00 - $150.00 per day
