Case Managers become a (CHW) Community Health Worker at Author Health

Are you someone who has a heart for helping others and a knack for technology? Author Health is looking for individuals like you to join our team as a Community Health Worker (CHW) to improve the lives of seniors with mental health and medical conditions.
What You'll Do:
As a CHW, you'll be the bridge between patients and their clinicians. You'll visit patients in their homes or community settings, or connect with them virtually. Your goal will be to help them understand and use technology to meet with their healthcare providers, ensuring they receive the care they need.
Your Key Responsibilities:
- Build Relationships: Form trusting relationships with patients to support their health goals .
- Tech Setup: Help and teach patients how to set up and use technology for virtual healthcare appointments .
- Care Coordination: Ensure patients attend their medical appointments and follow up as needed .
- Health Education: Teach patients about managing their health and navigating the healthcare system .
- Advocacy: Serve as an advocate for patients, helping them communicate with healthcare providers and access necessary services .
- Outreach: Engage in outreach activities to connect with target populations and promote wellness .
Who We're Looking For:
- Empathetic and Non-Judgmental: You have the ability to listen and empathize with patients from diverse backgrounds .
- Tech-Savvy: Comfortable using technology and guiding others in its use .
- Detail-Oriented: Attentive to documenting care interventions and patient interactions .
- Community-Minded: Knowledgeable about community resources and eager to connect patients with them.
Qualifications:
- High school degree required; further education is a plus .
- Bilingual abilities are preferred.
- Valid driver’s license and access to a car.
- At least 2 years of experience as a CHW or worked in a healthcare or caregiving role.
- Ability to work in a HIPAA-compliant, distraction-free environment for calls.
Job Type: Full-time
Pay: $21.00 - $22.00 per hour
Expected hours: 40 per week
