At the highest level of, OpusCare of Texas Employees must be committed to hospice philosophy and mission. Provide outstanding customer service to both internal and external customers. As a member of the Interdisciplinary team, the “Field Nurse” works under the direction of the “Supervisor of Nursing” and/or in her/his absence the “Nursing Supervisor”, and the patient’s/Hospice physician in the provision of skilled nursing care & services, assessment, planning and care in order to maximize the comfort and health of patients and families. Responsible for providing nursing care and services, assessing and identifying patient/family needs, and providing skilled care and supportive services in accordance with the patient’s physician’s orders and plan of care and OpusCare of Texas policies and procedures.
Essential Duties
At least two years of varied work experience as a professional nurse, and preferably one of the two years in an acute setting, home health or Hospice setting. Available to work across the State of Texas. Demonstrate nursing practice skills and the ability to assess and respond to the needs of patients and families in varied settings and the ability to execute a supportive, sensitive approach to patient/family/personal caregiver (PCG) or significant other (S/O) needs. Have excellent communicating skills, both orally and in writing, to include the ability to liaison all aspects of patient care/services, obtain cooperation from co-workers and other services sources, as directed or so required. Have the ability to work diligently and become knowledgeable and adhere to program policies and related procedures and of pertinent State and Federal Hospice regulations. Have the ability to cope with patient/family/personal caregiver (PCG) or significant other (S/O) emotional stress and be tolerant of individual lifestyles.
Responsibilities
Assume nursing responsibility for a patient/family caseload that includes the assessment, planning, implementing and evaluating phases of the nursing process. Initiate, and relate to the office staff, communication with attending physicians, other Hospice staff members, and other agencies, as needed, to coordinate optimal care and use of resources for the patient/family. Maintain regular communication with the Team Secretary and Team Director to review caseload. Maintain regular communication with Team Director/On-Call Supervisor to review the daily caseload and patient’s disease progression/reports, as necessary. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support. Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, or by providing direct care, as appropriate. Meet regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support and exchange feedback aimed toward enhancing professional growth. Attend interdisciplinary team conferences and patient conferences to facilitate discussion of issues from caseloads for full staff discussion, consultation, and evaluation. Provides information to department director(s) of unusual or potentially problematic patient/family issues, assisting during the problem resolution phase and providing appropriate follow-up intervention.
