The Roland Center (formerly San Gabriel Valley Training Center) offers high-quality rehabilitation, vocational training, job placement and work support services to the developmentally disabled. Our mission is to provide and promote opportunities for adults with developmental disabilities to live, work and interact in the community and achieve their personal best. Today, The Roland Center serves nearly 300 adults ranging from age 22 to 80 at its two center work centers.
Basic Purpose
This role will be directly working with individuals with developmental and intellectual disability.
The Case Manager is responsible for working with participants to ensure that they can be integrated to The Roland Center services. Their duties include meeting with participants and their families, service coordinators, and maintaining consistent communication with participants to assess their wellbeing and make changes as needed. Case managers are also responsible with communicating with Regional Centers, Department of Rehabilitation (as needed), and other referring agents on a constant basis.
Responsibilities Include

Work with referring agents (Regional Center, DOR, etc.) and manage participant intake

Ensure participants are integrated to the respective programs as assigned

Manage participant’s case file and ensure confidentiality in accordance with HIPAA guidelines
-
Assess the participant’s behaviors and takes note as necessary

Manages participant’s problematic behaviors and communicate issues with referring agent
-
Communicate with participants on a regular and constant basis
-
Establish individualized goals for each participant
-
Assess participant’s physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
-
Listening to participant’s concerns and providing counseling or intervention as needed

Recording participant’s progress including charting referrals, and other notable interactions
-
Evaluating participant’s progress periodically and adjusting as needed to improve outcomes

Submits the necessary and pertinent documentation to the program manager on a constant and timely manner

Maintain annual and semi-annual reporting to Regional Center

Creates SIR (special incident report) in lieu of work-related accident/incident and submits the report to Regional Center
-
Work with the program director to ensure that the program meets CARF requirements
-
Documents the work output of each participant (piece rate, productivity, hourly rate, etc.)

Analyze the report created by production supervisors to verify accuracy

Deals with work-related accident/incident by providing necessary first aid as needed

Handles participant conflict or incidents with diligence, and discretion

Maintain open communication with participant’s support network

Ensures participant authorizations are up to date and current to avoid delay or work stoppage

Respond to work site memos as needed and as instructed by the program manager

Monitor any work-related concerns, updates, or issues and communicates it to program manager

Ensure a positive work environment for all participants

Conduct site visits on a regular and constant manner

Performs other tasks pertinent and necessary to the role
Requirements:
Other Requirements

BA in Social Sciences or healthcare related field
-
At least 2 years of proven work experience as a case manager or similar role

At least 2 years of proven work experience in the field of rehabilitation or similar field

Must have experience in dealing with individuals with developmental and intellectual disabilities
-
Knowledgeable in behavior management and creating individualized plans
-
Knowledge with basic ASL highly preferred but not required
-
Demonstrate knowledge of the Regional Center and Department of Rehabilitation’s service delivery system
-
Must have reliable transportation and comfortable with commuting to La Puente office and Covina site (as needed)

Able to lift at least 25 lbs. as needed

Possess excellent time management skills

Must be able to maintain a professional and positive demeanor

Proficient in technology, including Microsoft office

Ability to read and comprehend simple instructions and memos

HIPAA Certification required or obtained within 30 days from start of employment.

Active First Aid and CPR Certification required or obtained within 30 days from start of employment

Must successfully pass mandatory background check, drug test, and medical examination
