ABOUT US:
High Watch is a non-profit dedicated to healing those suffering from the chronic illness of drug and alcohol addiction. Each resident receives state-of-the-art, customized medical care while learning the lessons on how to stay sober through the spiritual teachings and practice of the 12 Step principles of Alcoholics Anonymous.
JOB SUMMARY:
The Clinician is responsible for providing direct clinical services. The clinician also maintains a safe and efficient working and treatment environment per company policies, procedures and best practices; communicates effectively with the treatment team to ensure safe, quality care is provided to all guests.
JOB DUTIES:
- Provides direct support services to include engaging guests in Individual and Group Therapy, Family Therapy, Case Management, health and wellness promotion, and crisis management.
- Conducts assessments or screenings with guests to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals.
- Develops and provides individual treatment plans; monitors, evaluates and documents guest’s progress.
- Holds responsibility for intake documentation, treatment planning, and progress notes, group notes assessments and collateral contacts. Prepares documentation regarding guests in accordance with regulatory requirements and company policies and procedures.
- Collaborates in conjunction with Continuing Care and Psychiatric providers as part of a treatment team
- Monitors case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, case reviews and discharge plans and ensures all modalities are clinically appropriate.
- Facilitates coordination of care to include case review, therapeutic interventions, and guest progress in treatment
- Completes all other tasks as assigned
MINIMUM QUALIFICATIONS:
- Master’s Degree and license eligible in Connecticut in a field related to Behavioral Health, this includes: LCSW, LMSW, LADC, LMFT or LPC. Connecticut licensure to be obtained as soon as feasible after hire.
- Preferred experience with psychiatric/substance abuse and interest/experience in dual diagnosis treatment.
- Familiarity with 12 Step Program is helpful.
