Job Summary:
Position Summary: Responsible for accurate completion of MDS 3.0, review of care plan and accurate documentation to support MDS assessments. Provides leadership and clinical assistance to all nursing personnel to ensure facility’s conformance with nursing care standards as applies to RUGS.
Qualifications: Valid and current NYS RN license required. BSN preferred. Minimum of 1-3 years of LTC geriaitric nursing experience. Must be able to read and write English and have good oral and written communication skills.

MDS and Care Plans:
· Completes MDS assessments as per MDS schedule of their assigned unit.
· Routinely reviews each MDS to ensure each discipline had completed their applicable sections, and have signed off with their name, section and date upon completion.
· Routinely communicates MDS/CCP completion problems and chart issues via the Chart/Care Plan Follow up form and distribute to the discipline’s specific Department Director, as well as the Director of Nursing; and if necessary, to report trends of untimeliness or errors to the Administrator.
· Chairs CCP Meetings ensuring all disciplines review and completes their assigned care plans as per Care Plan Review schedule.
· Creates all UDAs for all OBRA MDS assessments by ARD and monitors completion. Communicates to all responsible disciplines all that are not completed timely.
· Attends Morning Report daily with ongoing review of 24- hour report to identify changes in resident’s condition for capture on MDS once MDS scheduled.
· Tracks residents’ status and condition daily via the Significant Change forms submitted by disciplines, 24-hour report, Weight Loss Monitoring Log from Dieticians and to evaluate if there is a significant change in status. If so, an MDS assessment will be scheduled accordingly.
· Reviews all MDS scores prior to submission and corrects MDS as necessary to ensure the most accurate RUG category is captured. Comparison and review of prior MDS RUGS score to current MDS RUGS score needs to be done before locking MDS.
Medicare Responsibilities:
· Distributes list of Residents on Medicare with appropriate skilled care needs (Benefit Days Remaining Report).
· Completes Notice of Medicare Non-Coverage forms for Medicare discontinuations and Benefits Exhaust letters for medicare days exhausted, as well as, contacting resident or designated representative.
Audit Responsibilities:
· Performs chart review audits, unit rounds audits and meal audits prior to survey and assists in the survey process on assigned unit.
· Assists in QA MDS audits monthly/quarterly.
Other Assigned Duties:
● Performs nursing rounds as needed.
● Assists with nursing staff shortages as needed.

Safety and Sanitation:
· Follow all established safety procedures and precautions when operating equipment.
· Report equipment malfunctions or breakdowns to the program manager as soon as possible.
· Report all unsafe/hazardous conditions to the program manager immediately.
Customer Service:
· Maintains an adequate liaison with families, residents and patients.
· Demonstrates awareness, respect and concern for internal customers (coworkers) and external customers (i.e. patients, residents, families, physicians, referral sources)
Residents' Rights:
· Maintains CONFIDENTIALITY of all resident care information to assure resident rights are protected.
· Assures that the resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.
