We offer treatment for conditions such as anxiety, depression, ADHD, PTSD, and co-occurring disorders. What You’ll Do
This is a full-time role for a Mental Health Case Manager. As Sol’s Case Manager, you will be responsible for providing case management services to clients, including orienting new clients to the program, assessing their needs, developing and implementing treatment plans, coordinating care for ancillary services, and occasionally performing group therapy. You will work closely with a multidisciplinary clinical team to ensure the best possible outcomes for our clients. This is an on-site role located at our facility in historic Downtown Riverside.
- Provide case management services to designated caseload (5-10 clients per week)
- Meet with clients at least once per week to build positive relationships and connection
- Create a daily and weekly structure for clients
- Provide client’s family with updates throughout treatment
- Be sensitive to cultural issues, past trauma, and other sensitive client issues
- Document progress throughout client stay in treatment
- Transport or arrange for client transportation to and from the facility and appointments
- Coordinate care as needed with therapist and other clinical staff
- Empower client’s ability to function independently through the delivery of ancillary service referrals and coordination of care
- Collaborate with client to create a comprehensive aftercare and discharge plan
Who You Are
- Passion for helping others achieve their personal goals in the context of mental health treatment and recovery
- High School degree or GED
- Experience in providing case management services
- Strong interpersonal and communication skills
- Ability to work effectively in a small team
- CPR and First-Aid Certification preferred (not required); company sponsorship available
- Associates degree or Bachelors degree from a college or university preferred (not required)
- At least 1 year experience working in the Mental Health or Substance Use Disorder industry preferred (not required)

Pay: $20.00 - $23.00 per hour
