Position Overview: Nu Care Inc. is seeking a compassionate and efficient Clinical Care Coordinator to join our team. The Clinical Care Coordinator will play a vital role in coordinating and managing medical appointments and services for individuals residing in our Community Integrated Living Arrangements.
This position requires compliance with all of Nu Care’s standards that are in alignment with the Illinois Department of Human Services, including all policies and procedures. Compliance is a part of the regular performance evaluation for Clinical Scheduler .
Responsibilities:
- Ensure that all appointments are made within DHS guidelines
- Coordinate and schedule all medical appointments, doctor visits, therapy sessions, medical tests and all other appointments for individuals
- Maintain accurate and up-to-date records of appointments, labs, and client preferences on a continuous basis to ensure compliance.
- Maintain accurate schedule and visit information
- Properly file individual records in their respective charts in accordance with Nu Care’s policy and procedure.
- Collaborate with healthcare providers, individuals and their families to confirm and reschedule appointments as needed.
- Communicate effectively with healthcare providers, onsite nurses, DSP manager and care teams to ensure the on-going needs of individuals are met.
- Respond to appointment-related inquiries with urgency and efficiency.
- Collaborate with healthcare providers to ensure that necessary documentation is accurate, properly prepared, signed by the appropriate physician and/or appropriate staff as needed and available for each appointment upon individuals set appointment date.
- Promptly address scheduling conflicts and adapt to changing needs, while prioritizing individuals well-being.
- Make informed decisions in meeting the individual’s healthcare needs
- Update scheduling calendar with annual, semi-monthly, quarterly and any upcoming appointments
- Ensure documentation is accurate and completed in a timely manner
Qualifications:
- Highschool diploma (Required)
- Being able to work independently
- Experience with medical terminology and the ability to read, understand and follow doctors orders.
- A sense of compassion and empathy for working with individuals with disabilities
- Proven experience in appointment scheduling.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Multi-task in a fast-paced environment.
- Proficient in Microsoft Suite, Excel and Word.
- Maintain confidentiality and safeguard consumers information.
- Problem-solving skills in order to manage scheduling conflicts.
- A strong commitment to improving the lives of individuals with disabilities.
- Excellent follow- up skills
