As the Care Coordination Supervisor, on any given day you will:
- Accomplish Health Home program objectives by supervising staff and organizing and monitoring work processes;
- Engage with staff in meaningful conversations about professional development, workplace culture, and program/agency priorities;
- Work with staff and clients to achieve optimal client and programmatic outcomes;
- Act as a liaison between the Housing Works Health Program as important internal and external stakeholders;
- Have the opportunity to work on myriad of exciting projects aimed at improving staff and client experience and outcomes
Qualifications:
- Master’s Degree in Social Work, (LMSW), Nursing (RN), Psychology, MPH, Human Services or other related field required
- Minimum two years supervisory and administrative experience
- Ability to supervise, manage and provide leadership to all staff members and trainees
- Adept with word processing, Excel, and Microsoft Office products; Previous experience with Electronic Health Records a plus
- Ability to communicate well both written and oral
- Good organizational and multi-tasking skills
- Strong analytical skills centered around collecting, organizing, and disseminating information
- Positive attitude and adaptable with both staff and clients
- Highly organized with the ability to work independently and prioritize multiple responsibilities in a fast-paced environment
- Comfortable working with a diverse group of staff and clients
- Ability to travel within the boroughs via public transportation as needed and ability to work from remote office locations.
- Requires access to email, phone, Zoom and Teams.
- Requires ability to comply with agency confidentiality guidelines in remote workspace
