Description:
Purpose
To provide professional direct resident nursing care and, in conjunction with the Charge Nurse, to supervise daily nursing activities performed by Nursing Assistants. To promote and maintain a positive physical, social, and psychological environment for residents in the Skilled Nursing Facility. To perform these duties in accordance with applicable current federal, state, and local regulations and standards, and as may be directed by the Charge Nurse; and to perform these duties in accordance with the mission of Elm Terrace Gardens to promote the health and well-being of individual residents based on their functional abilities and potential.
Nursing Care Functions - Duties

1 Render and administer professional nursing services, including the administration of medications, in accordance with physician's orders and the physician-approved individual resident care plans; and provide direct nursing care as necessary to assigned residents.

2 Admit, transfer, and discharge residents as required; as directed, ensure that rooms are properly prepared to receive new residents.

3 As directed, notify families or responsible parties about resident expirations or transfers to acute care facilities.

4 Make rounds with residents' physicians as necessary and ensure their completion of record keeping.

5 Consult with physicians in providing care, treatment, diagnostic, and therapeutic services.

6 Notify physicians of incidents and changes in conditions.

7 Accurately complete, maintain, and verify drug inventories, medical forms, physician and consultant orders, reports, evaluations, and resident chart entries according to established policies, procedures, and guidelines.

8 Knows and complies with the facility infection control policies and procedures, including the blood- borne pathogen standard, infectious waste disposal, and such other standards as may be promulgated from time to time.

9 Ensure cleanliness and safety of all nursing service and treatment areas.

10 Follow established safety rules and regulations at all times, including fire safety procedures and disaster control.

11 Promptly report accidents, incidents, and unsafe and hazardous equipment or conditions.
Requirements:
Qualifications
1 Must be a Licensed Practical Nurse in good standing and be currently licensed by the
Commonwealth of Pennsylvania Bureau of Professional and Occupational Affairs, Department of State; or
2 Must be a Licensed Practical Nurse in good standing currently licensed to practice in another state with a temporary permit to practice in the Commonwealth of Pennsylvania, and must obtain Commonwealth licensure within twelve (12) months of employment; or
3 Must be a graduate in good standing of a Commonwealth-approved school of practical nursing with a temporary permit to practice in the Commonwealth after passing the state board examination; and
4 Must be knowledgeable of nursing and medical practices and procedures and laws, regulations, and guidelines governing skilled nursing facilities; and
5 Must be at least eighteen (18) years of age and completed high school or obtained the equivalent of a high school education; and
6 Must possess the ability to speak to and be understood by residents and staff, and demonstrate the ability to read, write, and follow oral and written directions; and
7 Must possess an agreeable and helpful attitude toward senior adults.
