SOCIAL SERVICES DIRECTOR – Full-Time - Generations at Regency, Niles, IL
Generations at Regency in Niles, Illinois Seeks a Dynamic Social Services Director. This is a Full-Time role with a Monday-Friday, 8:00am - 4:30pm Schedule, and an Annual Salary of between $65k and $70k, Depending upon Experience.

Generations at Regency is a premier skilled-nursing community in Niles, Illinois, located on the banks of the Chicago River. Responsibilities of the Social Services Director:

Assist with planning, developing and directing our facility’s social service programs in accordance with current federal, state and local standards, guidelines and regulations

Complete all care plans

Complete the MDS (Minimum Data Set) within the Assessment Reference date
-
Ensure that medical, emotional, and social needs of each resident is met/maintained
-
Greet and orient new residents
-
Provide information regarding community resources to residents' families
-
Direct hospice and palliative care
-
Coordinate discharge planning
-
Facilitate regular care plan meetings with residents and their families

Address residents' grievances and assist nursing or other pertinent departments in resolving issues

Requirements:
of the Social Services Director:
-
Master’s Degree in Social Work (MSW)
-
MDS experience
-
3 years' social services experience
-
Experience in a Skilled Nursing facility
-
A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW)

Demonstrated knowledge of the social dynamic of the geriatric population
-
Knowledge of community resources
-
Initiative to seek new methods and services for residents
-
Strong leadership and relationship building skills
-
High level of organizational professionalism
-
Working knowledge of government regulations as relevant to social services
