SUMMARY
The Client Care Coordinator, under the direction of Social Work Supervisor (Clinical Coordinator), is responsible for the following goals: Enhance the delivery and coordination of mental health and related services to families with children in shelter; Promote and model best practices for shelter social service staff; Improve linkages to mental health and community based services; Increase the ability of shelter social service staff to address mental health issues in culturally and linguistically sensitive manner that incorporates strength-based, family-driven and youth/child-guided care; Strengthen overall permanency outcomes for families with children.
RESPONSIBILIITES:
Client/Service Delivery
- Complete a comprehensive bio psychosocial assessment with each family to understand strengths and services needs within 30 days;
- Facilitates a team review/discussion of the bio psychosocial assessment;
- Make referrals and appropriate linkages and helps determine the need for further intervention;
- Collaborates with ACS and/or preventive in ACS conferences to advocate for the family;
- Provides short-term counseling as needed;
- Provides crisis intervention when needed in conjunction with rest of social service team;
- Facilitate group work;
- Conduct risk assessments;
- Coordinates safety planning as needed;
- Prepares families and children to accept services;
- Communicate and collaborate with service providers to enhance engagement and ensure quality;
- Provide education/training to other staff;
- Provide psycho-education to families;
- Facilitates quarterly family meetings (with family + shelter staff) to discuss planning efforts and progress;
- Work from a strengths–based, family-focused, client-centered perspective and identify and fortify the strengths that each family has;
- Assist families with specific stressors associated with living in shelter;
- Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services;
- Confer and consult with professional and technical personnel implementing and multidisciplinary approach to client care and well-being;
- Serve as agency/program liaison to community agencies and/or groups;
- Develop linkage agreements with community based organizations, including local clinics in shelter catchment areas;
- Coordinate training for social service staff quarterly, which will be tailored to the individual shelter facilities;
- Performs other duties as assigned by Social Work Supervisor (Clinical Coordinator).
Sanctuary Model
- Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
- Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
- Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
Team/Staff Relations
- Work as a Providence House team member with all staff in order to implement the mission and core values of the agency;
- Attend regular internal and external agency meetings;
- Work as part of a team to plan and develop resident programs.
Administrative
- Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.;
- Documentation: SOAP notes, workshop summaries, Family meeting agreements;
- Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources;
- Assists with any and all data collection systems.
REQUIREMENTS
- Licensed Master’s Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC).
- Must obtain license within 3 months of hire.
- Two (2) years of experience working with mental health issues.
- Knowledge of child & adolescent development, parent child relationships.
- Strong expertise in strengths based, solution focused, family centered practice.
- Excellent verbal and written communication skills and the ability to work independently.
- Excellent interpersonal and organizational skills
- Competent technology & management skills, including proficiency with using Microsoft Office; and client database management.
WORK SCHEDULE
This a FT Position. Must be available to families seeking assistance at least five days per week, eight hours per day. Needs to be open to flex time to be able to meet residents when they are available which means some nights and weekends.
SALARY AND BENEFITS
Salary is $61,248.98 Annually.
