Essential Functions
The Nurse Practitioner (NP) will collect, analyze, and organize patient care within a Nurse Practitioner’s scope of practice. Provide bedside medical care for frail, homebound, geriatric patients in their home which will be a private home, assisted living, or long-term facility. Collaborate with all members of the patient care team including outside agencies, hospice, facility, staff, and family. Visits are scheduled efficiently, and hours are flexible. Visit notes are entered in the EMR within 3 days of the visit date. This position reports directly to the Medical Director. Current NP license required as well as reliable transportation.
Responsibilities
- Provides initial and ongoing evaluation of patient care either in-home or via tele-health
- Collaborates and communicates with all members of patient care team.
- Completes all electronic documentation for patient care in a timely manner
- Adheres to all PPS, Federal, State, and HIPAA regulations regarding patient care and confidentiality.
- Perform other duties as assigned
Qualifications
Professional degree from accredited institution
· Board Certified
· Active NP license in Colorado without restrictions
· Experienced NP or may the right candidate as a new NP
· Experience working in an EMR
· Computer proficiency to include Microsoft Office
· Reliable transportation, current driver’s license and vehicle insurance.
Experience Requirements
Minimum of 1-2 years clinical perioperative experience.
1-2 years’ experience in the Nurse Practitioner role preferred.

Job Type: Full-time
Pay: $110,000.00 - $125,000.00 per year
