Guardian Angel Homes is an award-winning assisted living community specializing in memory care. The ideal candidate for this full-time RN Director of Health Services position in Hermiston, Oregon is reliable and experienced in all aspects of resident care in a long-term care setting. The Director of Health Services is an integral part of the leadership team, reports to the Executive Director, and is directly assisted and supported by an experienced LPN and one or more Residential Care Coordinators.
Guardian Angel Homes is located in beautiful Eastern Oregon, home of the famous Hermiston Watermelons, Thursday night Farmers Market and the Columbia River. Hermiston offers a great small-town, family-friendly atmosphere with the bustle and activities of Washington Tri-Cities just 30 minutes away.
Clinical and Other Responsibilities:
- Provide clinical oversight and assist with medical and holistic case management for care services, outcomes, and quality metrics for ~60 assisted living and memory care residents.
- Collaborate and coordinate with outside care partners and internal interdisciplinary team members to ensure continuity of care.
- Develop and ensure accuracy of a written Service Plan for each resident that identifies the needs of the resident and indicates the specific cares to be given; routinely audit and implement Service Plan changes as necessary.
- Assess potential new residents and discuss findings with admissions team to evaluate appropriateness of admission (some local travel required).
- Maintain accurate records of all nursing and care activities.
- Delegate staff on appropriate care tasks in accordance with company policy and state and federal regulations.
- Participate in resident care conferences with family members and others.
- Participate in investigations and documentation of accidents, incidents, emergencies, and resident complaints and help determine and implement appropriate follow-up steps based on findings.
- Participate in and provide oversight for hiring, training, evaluating, disciplining, and terminating Nursing Services staff (caregivers, med techs, etc).
- Assist with determining appropriate Nursing Services staffing levels.
- Help ensure adequate levels of nursing supplies.
- Understand and ensure compliance with all laws and regulations pertaining to assisted living and memory care.
- Follow and help ensure compliance with infection control measures when necessary.
- Provide routine education, support, and accountability to staff to ensure consistent quality care reflecting GAH standards.
- Participate in identifying department areas for process improvement and provide education, support, and accountability to Nursing Services personnel to achieve goals.
- Participate as needed in all surveys, inspections, and other inquiries by authorized regulatory bodies.
- Maintain confidentiality of all resident Protected Health Information as indicated within HIPAA guidelines.
- Maintain confidentiality of staff information.
- Routinely available for after hours and weekend on-call nursing needs.
- Other duties as assigned by administration.
Experience and Requirements:
- Must have and maintain current RN license in good standing as specified by state agency or ability to obtain required licensure within state agency guidelines.
- Must have 3-5 year experience in nursing with at least 1 year in geriatric care.
- Must have 2+ years of leadership experience.
- Must have good computer skills.
- Must pass criminal background check in accordance with state requirements.
- Must obtain/maintain current First Aid and/or CPR certification as required.
- Must obtain/maintain current Food Handler’s Permit.
- Must be in good physical health (able to intermittently and routinely sit, stand, walk, bend, rotate, and conduct other physical movements inherent to practical nursing and able to occasionally lift up to 50 pounds) and demonstrate emotional stability
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability and willingness to deal tactfully and harmoniously with professional and non-professional personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must be at least 18 years old
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to practice continuous resident advocacy.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing practices.
- Must have the ability to function independently, have flexibility, personal integrity and knowledge to work effectively with residents, family members, staff, and other support agencies.
Hours: Monday - Friday 9am - 5pm (with some weekends and overtime depending on the need)
Pay: $80k-$110k DOE (plus on-call pay)
