Registered Nurse (RN)
Under the general supervision of the RN Supervisor, the RN is responsible for provision of direct nursing care and the supervision and guidance of nursing/PCW personnel in the implementation of nursing duties and responsibilities to clients and their families.
Duties and Responsibilities:
- Is responsible for the total health management of the client/family, providing direct nursing care and supervising and coordinating the services of others to the client.
- May make the initial assessment visit: regularly reassesses the client’s needs and problems on periodic home visits.
- Develops a plan of care for the client, giving full consideration to the clients’ preferences for service arrangement and choice of PCW, interpret the plan to the PCW.
- Initiates appropriate preventive and rehabilitative procedures for and with the client.
- Evaluate and provide clinical teaching or guidance as necessary to improve the service to clients or make referrals to other services appropriately.
- Secure written orders from the client’s physician.
- Renew orders every 3 months unless the physician specifies that orders covering a period of time up to one year are appropriate, or when the client’s needs change. Whichever occurs first.
- Develop appropriate time and service reporting mechanisms for personal care workers and instruct the workers on their use.
- Give PCW written instructions about the services to be performed and demonstrate to the PCW how to perform the services.
- Evaluate the competency of the personal care worker to perform the services.
- Participate with other community agencies and other agency personnel in promoting desirable community action.
- Promptly inform the physician and other personnel of changes in the client’s condition and needs.
- Do documentation and charting of all client care, in a prompt timely fashion.
- Maintains appropriate records, caseload management, and writes reports as indicated.
- Is able to do and gives nursing staff guidance on problem solving, motivating the family if possible, to assume some responsibility.
- Initiates client/family teaching demonstrations.
- Interprets agency policies, health, State and Federal laws, and regulations to staff, clients, and their families.
- May participate in operation of outreach activities and health screening.

Pay: $50.00 - $95.00 per hour
