Home Care RNs and Home Care Admissions RNs are responsible for client assessment, care planning, intervention and overall supervision of patient care and outcomes. Graduate of an approved nursing education program, licensure as a Registered Nurse in the State of Connecticut and have a minimum of one (1) year experience as a professional acute care nurse. Home care experience and accuracy in OASIS and developing a plan of care preferred. Waterbury, Hartford and Enfield, Full-time Days or Weekends.
