Position Title: Friendly Visiting Coordinator

Status: Full-Time
Compensation: $60,000 annually

Position Summary: CBN’s Case Management Unit serves homebound older adults in Upper Manhattan and Roosevelt Island and is searching for a searching for a Friendly Visiting Coordinator who will provide key client supports. For referred clients, the Friendly Visiting Coordinator will conduct telephone and in-home assessments to determine client eligibility, match clients with volunteers, maintain visiting schedules, oversee volunteers, monitor client needs, and work directly with referring Case Managers to maintain continuity of care. The Coordinator will also conduct outreach towards both volunteer recruitment and service utilization by clients. The Friendly Visiting Coordinator will work directly with the Director, Volunteer Services and Case Management Unit Program Director, and interface with organization staff and partners.
Primary Responsibilities:
Receive referrals from agency Case Managers
Complete comprehensive telephone intakes
Recruit, screen, train, supervise, and monitor volunteers
Conduct home visits to confirm client eligibility and complete a program assessment
Collect information about client’s interests, hobbies, and preferences for a volunteer and document discussion in case notes
Match clients and volunteers based on shared interests, compatibility, and geography
Inform assigned Case Manager of any additional client needs
Follow up on and document any concerns
Maintain a visiting schedule
Re-evaluate client’s need and appropriateness for the service on an ongoing basis, with feedback provided from the volunteer and the Case Manager
Attend the first visit with the client and volunteer
Contact clients every three months to ensure the match is going well and address any concerns
Meet with volunteers on a quarterly basis and as needed
Maintain compliance with NYC Aging regulations regarding service provision, deadlines, and documentation
Utilize online platform to maintain and review detailed client records including intakes, case notes, referrals, and client assessments
Participate in trainings/ meetings as required
Participate in weekly Case Management team meetings
Exhibit awareness of agency and community resources and effectively utilize both while working with staff and clients
Participate in case conferences as an advocate for clients as needed
Exhibit ability to incorporate Mission Statement philosophy in work with staff and clients
Exhibit ability to work well within agency and engage appropriately with co-workers within and outside of the Volunteer Service department and Case Management Unit
Accept other assignments, as appropriate, to meet the needs of the program and individual clients
Required Qualifications:
Bachelor’s degree with a minimum of 2 years of professional social work experience in the field of aging
Bilingual Spanish or Mandarin speaking preferred
Affinity for working with an older adult population
Passion and sensitivity for CBN’s mission
Strong written and verbal communication skills
Ability to work pro-actively, both independently and collaboratively
Strong proficiency with database/information management systems and Microsoft Office Suite
