This is a Per Diem Case Management RN role and you will be paid $40/hour.
- Make home visits to clients in designated geographic territories.
- Performs an assessment of a patient's physical, psychosocial, safety, and environmental needs on his or her first visit and reassesses them at each subsequent visit.
- Develops a written plan of care in accordance with the patient's diagnosis, and family needs, physician orders, and the corporate policies and procedures. Coordinates the development of this plan with other disciplines providing care for patients.
- Modifies the written plan of care as necessitated by changes in the patient's condition.
- Maintain compliance with recertifications every 60 days and post every inpatient hospital admission and supervision visit every 60 days for HHA and LPNs.
- Follow up with, execute, and properly document doctors' orders.
- Functions as a member of an interdisciplinary team, encouraging and maintaining open communications at all times.
- Case management and coordination.
- Functions in a cost-effective manner in regards to utilization of time, and supplies.
- Meets or exceeds established productivity and quality assurance standards.
- Demonstrates a professional attitude, sound judgment and continuity in interactions with physicians, other health care providers, and community agencies.
- Acquires and utilizes knowledge of community resources.
Qualifications
- One year prior clinical experience as a licensed RN is required
- Minimum Education - Associates Degree in Nursing
- Required Certification - Registration and current RN license in PA
- Ability to work independently and manage time effectively
- Ability to travel to cases as assigned
