Hushabye Nursery is an Arizona nonprofit organization with a mission to ‘embrace substance exposed babies and their caregivers with compassionate, evidence-based care that changes the course of their entire lives.’ The organization was founded by Tara Sundem, a neonatal nurse practitioner who witnessed firsthand the challenges of newborns exposed prenatally to opioids and those families trying to care for them. Hushabye Nursery’s innovative model offers a safe and inclusive space where mothers, family members and babies – from conception through childhood – can receive integrative care and therapeutic support that offers each child the best possible life outcomes. Programs will include prenatal and postpartum support groups, inpatient nursery services and outpatient therapies.
POSITION OVERVIEW:
Provides case management and direct staff support to foster stability and overall well-being of participant families.
QUALIFICATIONS/COMPETENCIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

In accordance with the vision, mission and values of Hushabye Nursery, the Family Coach is responsible for providing quality care and services that meets the needs of persons served.

Completes biopsychosocial driven assessments and intake forms with clients.

Maintain, and collaborate with client’s care team to ensure the creation of person-centered treatment goals for client’s treatment plan are being monitored, evaluated, and created based on client’s individual needs for services within the necessary agency timeframes

Conduct strengths-based needs assessment with each family on case load and develops family partnership agreements with participants, creating measurable goals to ensure families are successful in accomplishing their identified goals.

Provides care and services in a supportive, compassionate, and non-judgmental manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.

Collaborate with local programs and assist participants in accessing community resources and support services. Ensuring having up-to-date information and locating specialized resources as needed.

Actively Collaborate with Department of Child Services to assist participants in meeting goals and applicable requirements.

Timely and accurately complete documentation including utilizing an online database in accordance with the established guidelines, procedures, and HIPPA.

Coordinate, under Administrative Director approval, all financial requests.

Serve as an advocate for the participants (including attending court, Homeless Court, hospital and DCS meetings and writing letters of recommendation for participants as requested).

Inform and assist participants in enrolling into Hushabye programming.

Initiate and facilitates community partnerships.

Participate in teaching and facilitating classes for participants and support at program events/activities.

Maintain and uphold Standards of Practice.

Report child abuse and/or neglect as in accordance with Arizona State law.

Any additional projects or tasks as assigned by senior leadership.

May require flexible hours to include some evenings, weekends and/or on call coverage.

Completes required orientation and continuing competencies as required by policy and procedure.

Other duties as assigned.
Requirements:
KNOWLEDGE, SKILLS, AND EXPERIENCE:
-
Experience working with Medication Assisted Treatment, Opiate Use Disorder, Substance Use Disorder and Department of Child Safety preferred.

Expert knowledge of case management and healthcare management principles.

Experience working with transdisciplinary health teams.
-
Effective communication, organizational and time-management skills.
-
Background in health care, social work, mental health, and/or substance use.

Problem solving skills and ability to multi-task.

Must be passionate about treating families who are struggling with substance abuse.

Ability to help foster a trauma informed, inclusive environment for all mothers, families, and support systems.

Bilingual in Spanish preferred

Current AZ DPS Level 1 fingerprint clearance
EDUCATION (MINIMUM REQUIREMENTS):

Bachelor’s Degree in Social Work, Education, or other related field. Equivalent experience in the above knowledge, skills, and experience, can serve in lieu of Bachelor’s degree.

2 years of experience in Social Work or other related field.
