Nurse Practitioner Job Description
Overview:
Join our healthcare team as a Nurse Practitioner to conduct health risk assessments and provide patient care. This is a hybrid role involving in-home visits and remote work. We are seeking dedicated individuals to contribute to our mission of delivering high-quality healthcare services.
Duties:
- Perform health risk assessments, including history and physicals, within patient homes or healthcare facilities.
- Document acute, chronic, and long-term healthcare issues accurately in the EMR system.
- Educate patients and families on chronic conditions, preventative care, and medication adherence.
- Collaborate with patients, coders, schedulers, and supervising physicians.
- Complete referrals to case management, nursing, pharmacy, etc., as necessary.
- Review patient records, interpret laboratory data, and assist in diagnosing medical conditions.
- Conduct ADL assessments, fall risk evaluations, depression screenings, BMI evaluations, and medication reconciliation.
- Participate in clinical meetings and compliance trainings.
- Perform other duties as assigned by supervising physicians.
Skills:
- Experience in diagnostic evaluation and dementia care.
- Proficiency in medical imaging and utilization review.
- Ability to work with individuals with developmental disabilities.
- Familiarity with ICU settings and acute care environments.
- Knowledge of standards in Level I Trauma Centers.
Requirements:
- Active and Unrestricted NP License in the relevant state.
- Board certification by ANCC or AANP.
- Ability to be credentialed by Medicare, Medicaid, and private insurance companies.
- Current DEA licensure with prescriptive authority.
- Possession of current BLS certification from the American Heart Association.
- Valid state Driver's License and Auto Insurance.
This position offers a flexible scheduling arrangement based on the NP's availability. The role involves conducting 10-15 visits per week on average. Candidates should be willing to travel within a designated radius for patient visits. This is a contract position until 12/31/2024 with potential for extension.
