ORGANIZATION: Weston United Community Renewal Inc. is a Not-for-Profit organization located in Harlem, New York. For over 30 years, Weston United has provided housing and supportive services to participants with history of chronic homelessness, mental illness, and substance use. Weston United employs over 100 employees who contribute to a rich and diverse work environment.
Position Description:
- Responsible for supervising the physical condition of the facility, which includes but not limited to weekly facility/apartment inspection, ensure completion of monthly fire drills and self-preservations as necessary and ensure that residents are living in a clean and safe environment.
- Provides preventive intervention individual to reduce unnecessary de-compensation, hospitalizations and relapse and clearly document all contacts and interventions in AWARDS; available for 24-hour crisis management of any residents or building issues; ensures the delivery and documentation of client services including, but not limited to, weekly service-linked progress notes, collateral contact notes, monthly summary notes, service or support plans, assessments, and consents.
- Ensures case records are current and accurate; provides oversight of medication monitoring and logbook documentation [in accordance with OMIG and OMH documentation ; responsible for the supervision of all Residence Counselors, providing weekly supervision and all mandated onboarding and probationary and annual evaluations; assists the Program Director with staff scheduling, meetings, training, new hire orientation.
- Assists with program’s fiscal responsibilities including but not limited to inventory of office and food supplies, petty cash, distribution of residents’ funds, program fee payment plans and money management documentation; responsible for managing the shifts by ensuring that all shifts are adequately covered daily and that the residents and facility needs are addressed in a timely manner; ensures the safety, maintenance and upkeep of the building and apartments through ongoing assessment of maintenance requirements and collaborate with Facilities Department regarding repair needs of individual apartments and common areas.
- Assists Program Director with ensuring compliance with contractual obligations, including facilitating appropriate admissions and discharges, maintaining census, incident reporting and completion of required documentation; attend internal agency meetings and trainings. In the absence of the Program Director assume all managerial responsibilities under the direct supervision of the Clinical Coordinator and/ or Area Director; performs other duties, consistent with the goals and objectives of the program, as assigned by the Program Director, Area Director, Clinical Coordinator, AVP, VP and/or SVP. Any other duties assigned.
Qualifications:
- High School Diploma, preferably Bachelors in the Human Services field, with a minimum 2 years’ experience working in a supportive residential program and/or with individuals with serious mental illness and homeless background; computer literate with knowledge of Microsoft Office, and spreadsheets. Knowledge of AWARDS preferred. Experience with individuals with forensic backgrounds. Knowledge of the justice system preferred.
- Excellent oral and written communications skills; excellent organizational and planning skills; ability to effectively interact and communicate with all levels of employees, clients, and community resources.
